Elbert/Lincoln HB Program

1451 Programs / Elbert and Lincoln County

Elbert and Lincoln Counties, located in East Central Colorado, developed the Collaborative Management Programs (CMP), or Interagency Oversight Group, through Colorado State Legislature HB1451. Elbert County started in 2004 and Lincoln County in 2010. These programs eliminate duplication in services by working with targeted populations and collaborating with Memorandum of Understanding partners. CMP supports youth and families by connecting them to resources and early intervention, promoting positive outcomes, and improving family life. Key services include Individualized Service Support Team meetings to connect families with resources, Alliance Afterschool programming to increase academic and social skills for middle school students, and Rural Intervention Specialized Case Management (RISC), which offers assessments, referrals, and pre-trial case management for the Colorado Youth Detention Continuum (CYDC) in Elbert and Lincoln Counties.

The Collaborative Management Programs (CMP)

The Collaborative Management Programs (CMP), also known as the Interagency Oversight Group in Elbert and Lincoln Counties, were developed through the Colorado State Legislature HB1451. The focus of the local groups was to create a process to help eliminate duplication when programs/agencies work with the targeted population that meets the systematic criteria. Elbert County began this process in 2004, and Lincoln County began in 2010. Each community works closely with Memorandum of Understanding partners, identified as the experts in their individualized fields. Through collaboration, agencies work together by discussing what they see as barriers, what barriers are in the community, how these barriers can be eliminated, and creating programming that fits the community's needs. Both programs support youth and their families by connecting them to resources and early intervention. We advocate collaboration among service providers to tailor the support that meets the unique and specific needs of each youth and their family. Our goal is to empower families to sustain a healthy environment of support for their children while avoiding involvement in the juvenile justice and human service systems. Our services aim to serve children and their families in a way that promotes positive outcomes and improves the family's quality of life. Collaborative Management Programs offer different prevention programs.

INDIVIDUALIZED SERVICE SUPPORT TEAM

Individualized Service Support meetings are utilized to connect families with appropriate resources and address the needs/concerns of youth and their families. These meetings involve discussions with Connections for Families staff and representatives from other local agencies to offer support and resources to meet these needs.

ALLIANCE AFTERSCHOOL

Connections for Families offers afterschool programming for middle school-aged students on Tuesdays and Wednesdays throughout the school year. This group is intended to increase academic engagement, social-emotional skills, and social connectedness within the youth's schools.

RURAL INTERVENTION SPECIALIZED CASE MANAGEMENT

Our RISC Case Manager works with youth and their families on several levels. The RISC program offers comprehensive assessments to evaluate areas of strength, need, and risk for a child/youth. The RISC case manager also provides referrals to appropriate services, assists in facilitating ISST meetings, and serves as a Pre-Trial Case Manager for the Colorado Youth Detention Continuum (CYDC) in Elbert and Lincoln Counties.
